What to Bring to Your Visit
Bringing the right information helps our team check you in and complete your visit more efficiently.
For most visits, bring:
- Photo ID
- Insurance card, if using insurance
- Payment method
- Medication list
- Allergy list
- Any recent test results, if relevant
- Parent or guardian if the patient is a minor
If your visit requires forms, bring:
- School physical form
- Sports physical form
- Camp physical form
- Work or pre-employment physical form
- DOT physical form, if applicable
- Employer authorization or workers' compensation paperwork, if provided
